Always when you feel that the plunger and the IOL don't go forward normally and easy in the loader and in the cartridge, think that it might be a problem with the IOL. Preloaded IOLs are susceptible to this complication, as well. Should you have any doubt, better avoid implantation in the AC, push the IOL out from the cartridge, examine it carefully at the microscope and, if it is broken or damaged in any way, change it and implant another IOL. When the IOL looks good at the microscope exam, you can choose either to implant it with another cartridge, or to change it.
Attention: these problems can not be seen, only felt at hand!
