Over the last two decades, Public Health-Seattle King County has demonstrated the unequivocal positive impact of a Community Health Worker (CHW) home visit model on chronic disease health outcomes. Through a series of successful clinical trials, this promising approach to provide home-based self-management support has been tested on Diabetes, Asthma, Hypertension and COPD. This year, the team has received a PCORI Award to disseminate and implement a CHW Asthma Home visit model in communities across Washington State and developing a training for managers who supervise CHWs.
The team at PHSKC is comprised of CHWs with over 13 years of experience each and dedicated administrative staff that provide supervision, training and support for the work. The team will work collaboratively with stakeholders statewide to train and support CHWs and CHW supervisors on this model. In this webinar, they will discuss the details of the online and in-person trainings and collaborate with participants to brainstorm ways that the training can serve programs in communities with diverse needs.
